I am also experiencing the same issue on a number of websites.

One Website has version 5.1.9.1 installed, another website has Version 5.2 installed.

Both sites have the same issue, as soon as ‘[event_form]’ shortcode is added to the page it only displays the Event name input field.

Can you help? Or is there a way I can roll back to previous version of Events Manager?

PS. this plugin is fantastic and this is the only issue I have had with it.
