your form should be like below form:

when I go to edit of downloads, then from the TOP Screen of Editing, file url is “https://agilecoachtools.com/download/416/&#8221;

and downloads works, now can you tell me the problem please ?

I am using contact form 7 and download monitor,and all plugin is latest version,my contact form is below, see:

and now here is my short code for homepage:
