@champsupertramp thanks i got it already, but i want to ask if possible i can auto display Form ID at the top of the registration form?

I mean with each click on the registration form link, it should auto generate a unique ID and display it at the top of the same registration form.

Now i want with each click on that link, the system auto generate a form ID or a unique ID and display it at the top of that form so the application can copied and save it.

I want a system where users can login and fill a form on my wordpress site, then make payment. admin then approve their form and they can print out to use it as a gate pass or ticket for an event.

NB: users should be able to upload their passport photo in the box above on right hand

i still cant get to with the UM form builder. cant i use any other wordpress plugin to just create that type of form?

@tenzinchoenyi thanks for the reply. if i used contact form 7, how do i integrate it into UM? i want a link to be available in the member profile tab where users can click and take them to fill the form. please any idea how i will achieve this?

still cant locate the file under my host server when i login in, is the file going to be under wp-adin, wp-content or wp-include, plz need ur urgent help.
