In reply to: “Non-existent changeset UUID.” errorI deactivated the extensions, logged out, deleted the entire site and re-uploaded WordPress 4.7.3, the single theme and the extensions, which were subsequently enabled. The Non-existent changeset UUID. error problem persists. I’m open to all suggestions.
For reasons unrelated to the contact form extension, I elected to delete and re-upload my WordPress site. In the new install, the contact form error message was suppressed.

In reply to: “Non-existent changeset UUID.” errorI deleted my secondary site and created it all over again, again as a (virtual) subfolder. I was able to preview the site correctly as I rebuilt it. However, the preview was replaced by the Non-existent changeset UUID. error when I went into Network Admin | Settings | Domains and added the domain to my site using the domain name and the Site ID. If I remove the site from the network, I can again preview the pages as I work on them. I strongly suspect this is a WordPress bug. Please confirm and/or provide a way to avoid it. It would be nice to be able to preview a published site.
I used phpMyAdmin Search to remove or update all references to the outdated shortcode [contact_form]. I added the custom code you provided, activated it and saved the changes. I refreshed my web site and visited each of the four pages. Nothing was detected. I still see the warning about outdated shortcode. I strongly suspect the warning is generated by a bug. Can you please confirm?
