@hereswhatidid was right. I seem that you need to Resave that settings if any change to .httpaccess is done, manually, of by a plugin like MathRank SEO plugin. Just resave and it works again. Thank you.

This is for “Contact”. I only get “Fax number”, but not “Phone number”.

Update: it worked if I skipped the initial “tutorial”.

Update – you can change the default “10” per page in the plugin file “contact_form_listing.php” in the line 124 where is the variable $show_record = 10 to whatever you need. I changed it to 200 and it is perfect now.
