It works now! I didn’t think that would be the issue since I’ve used numbers in slugs on other forms. But apparently, the numbers can’t come at the beginning of the slug.

When I look at the Debug Log for my test submissions, I see this:

[ERROR_MSG] => Error in Google Request

I actually had that option on originally when it didn’t work. I unchecked it and it still didn’t work. Now I’ve checked it again and it still isn’t working. Other ideas?

Yes. In the Google form, dashes are used instead of underscores.

Honestly, I don’t know what changed. I was going to change it a few days later to include all the FAQ information without using FAQ Concertina, but then it started working, so I just left it. Wish I could identify what was different. I didn’t add or take away anything on the page. I had disabled some plug-ins a couple days beforehand, but it didn’t seem to make a difference at the time.
