Yoast tells me my keyword does not appear in the first line when it does.

Yoast is giving me some errors that are incorrect.
Yoast says
“Focus keyword usage
Your focus keyword was found in:
Article Heading: No
Page title: Yes (1)
Page URL: No
Content: Yes (4)
Meta description: Yes (1)”Now my focus keywords are
Fun + Quirky Wedding Photographer San Diego Las Vegas?New YorkMy page title would look spammy if I gave it the same title.
The page URL DOES have the keywords, but yoast says it doesnt.

Yoast also says I don’t have it inside of <h2> but I do.
Before I forget, I read this thread that is closed, and did not understand what the last guy wrote.
