WPUF Form not displaying correctly

Hi, I have 3 Questions

q1) I have just created a form using WPUF and pasted the shortcode to the new page. However the font and layout of the form is not appropriate i.e. as it should be. The “category box” has shrinked to a size where the “select” word inside the box is now dsiplayed vertically. Any idea whats wrong?
Everything is on default by the way

Q2) How do I add categories to WPUF form ? It gives me the option to add “Exclude Category ID”, but doesnt ask what category do i need to put the post in?

Q3) Where do the posts go after being submitted? Do i need to create a seperate page for it? If so whats the shortcode for that?
