[WPCR_INSERT] not working

In my page, I am using
[WPCR_INSERT]
I have enabled “Enable WP Customer Reviews for this page”.
It shows the form and it shows the reviews. I only want it to show the form and no reviews.
You can see it in the sample images the author uploaded.
How do I fix this?
