WP Form with "Add another" capabilities?

  • I’m trying to create a form that can collect a list information about various individuals. For example, someone would fill out “Name, Gender, Age” and then click “Add another”.

    Is there any form that allows for this kind of data entry? We would need to collect an unlimited number of this type of info.

    The other alternative is to just provide a spreadsheet that someone could fill out and upload.

    Any suggestions?
