wp contact form help

Well I tried to plugin a form that was modded by a doug karr. Well the plugin did not work at all for me. So I tried to remove it and install the previous version but then that was corrupted. Well now I can’t activate the plugin because it causes a fatel error. I tried other plugins that would activate but to no aveil the did not work. I did modify my side bar and index file but changed them back to the way they were when I was done trying to install and activate another contact form. Basically the code is not working and I am left with no contact form.
Please some feedback any would help. Any takers?
