Works great. Simple to use. Workaround for no phone number field

The only thing that I wished this had was a field for an optional phone number. I found that replacing “Your Message” with “Your message – please include phone number if you would like us to call you.” in the message placeholder field works fine. I know that there is a way to add another field, but it is beyond my capabilities at this point.
