Works great!

After trying half dozen form plugins, this one scored big. All the others left too much for me to figure out. This one has documentation and helpful videos to walk you thru how to use it. Very happy. If my requirements ever become such that I need greater functionality, I’d be very comfortable upgrading. Great plugin!
