WordPress site hacked

For past 2 months my wordpress sites are being hacked I changed themes deleted plugins updated wordpress installation.

I don’t know how hackers write codes inside my wordpress files example in wp-config.php they put this:

On top of my wordpress files they put this code I don’t know how they can write into it. I have tried all possible things reinstalled wordpress deleted all plugins changed themes. Is this really a wordpress problem or a server problem.
