favicons tend to be aggressively cached by browsers. To see whether this is the case try to refresh it by entering the full URL to the favicon file in the browsers address bar and force a refresh using [ctrl] + [F5]

I doubt it is a WordPress or theme fault, in my experience this most likely caused by your browser.

Have you viewed the site on different browsers & different computers?
    What theme are you using?
What is the URL of your WordPress site?
How have you changed the favicon?
What does the required favicon look like?
What does the old one you are still seeing look like?
