WordPress and Map integration

I have been trying out wordpress and google map integration but am stuck at a point.

Single location plotting works fine, but when I try multiple location plotting I fail ??

What I need is this –

I have say 5 custom fields
Address_1,Address_2,Address_3,Address_4,Address_5

I want it to run through a loop like if address1 then add to array…

finally build an array of addresses based on addresses above and then plot them on a map with an interactive map which could plot location one after another with directions like a route map.

any ideas / plugins to do this and how my issue can be solved
