WooCommerce shipping not showing both shipping options

I have had issue with this before and have since had a couple of site updates of various things. So I have two shipping options setup – first class large envelope and medium flat rate box. This covers about everyone that buys from me nicely. I noticed today that they first class envelope has dropped off of the shopping cart. I have the setting checked in woocommerce shipping setting for first class envelope and for medium flat rate box.
