with 4.0 not working

    If I could get a few more steps to reproduce, on a clean install it seems to be working fine for me. If you can try it without any other plugins installed that’d help a lot, if not if I could get a little more information about what you did/what you expected to happen. From what I get from your post it just added the image to the post editor and not to the attachment section.
