Wiki with customizeable forms

  • Does anyone know of a wiki plugin that will actually insert forms into a post?

    I’d like to start working on a database that has author, lyrics, key signature, etc. for some songs. I’d also like my users to be able to easily edit that information.

    Basically, I’d like to be able to add new records (posts) from the front-end that have specific fields for my users to fill out. I don’t really think having people edit records through the dashboard is a good idea.

    I’ve looked for months and have yet to find something that meets those requirements. Any suggestions?
