Widgets not working with one form but are with another

I am managing a site for a friend https://www.dreamscoffeelounge.co.uk and have two widgets in the right hand column for his Tripadvisor ratings. One is for a certificate and one is the rating form to fill in. Both widgets work fine on all pages except the “Afternoon Tea” page where a place holder for the widget loads but the rest of it doesn’t. If I remove the form from the page the widgets load. If I put the contact form (from the contact page) on the afternoon tea page the widgets load. But with the “booking form” on the page the widgets don’t load.
It has to be some interaction between that particular form and the widgets but I have no idea what. Your assistance would be greatly appreciated.
