Widget stopped working after server migration

Used this widget on a bunch of clinic location pages. The widget stopped displaying the map after a server migration. Nothing else changed. I thought it might have been a problem with the API key, so I made a new one, but it’s still not working. Any ideas? Other things broke during the migration, but I don’t know what I can do to get this working again. Thanks.
