Widget only shows title (no form)

I created an embed in Hustle (free version), and added the widget to my posts. I took screen shots of the widget placement screen and of the post where only the widget title appears but don’t see a way to attach them to this question.
The embeds I created and put in posts appear just fine, just not the one I’m trying to use as a widget.
How can I get the full embed (email sign up form) to appear as a widget at the bottom of my posts? Thanks.
