Widget is installed, does not show up on the front end

I have the widget installed and have added it to a page via the widgets panel under Appearance, filling everything out in the widget’s fields.

However, on looking at the page, via looking at the front end, the source code, and running it through Google’s structured data testing tool, I can see that the plugin is not on the page. Do you have any idea why this may be happening?
