Widget doesn't work, server settings empty

the server settings pages are all blank, can’t set the server to ‘active’ under settings – there’s a checkbox, but no ‘save’ button? Not sure if that’s important because there’s no documentation.

Setup a campaign, added the banner, it says that it’s active, seems to be fine in the admin, but the widget doesn’t display anything – I can see the ‘wrapper’ html for the widget, but no ad code gets displayed on the page.

Any ideas?
