Which file to edit to align radio buttons horizontally

I would suggest first trying to turn off “use included CSS” under the survey options – with that disabled, your theme’s CSS will control the layout – if that doesn’t provide the desired results, then you will need to create custom CSS to achieve the layout that you desire. Generally speaking, most themes provide a place for custom CSS but if yours doesn’t you might consider installing Jetpack as it does provide a place for custom CSS.
