Where's the subscribe button!?

I have a feeling I’m being really, really dense here but I’m receiving the preview emails I send to myself BUT I can’t for the life of me discover how to actually add a “Name”, “E-mail” or “Subscribe” button to the actual form!?

It just comes up with whatever title I’ve put in and then “manage subscription options from your dashboard”. There are no boxes for people to enter their details and hide subscribe!

Am a complete novice at all of this and, as you can tell, have no real idea of what I’m doing!
