Where do the field names come from

I have forms with entry fields named:
your-name
your-company
your-email
your-phone
your-message

The plugin spits CSV column names out that are very different:
Form_date
Name
Company
Email
Phone
Message

Can you explain the logic here; it is very confusing to me.
