Where can you download Simple Cart Customer Input addon

    Hi I have read the docs that say
    -Go to the Add New plugins screen in your WordPress admin area
    -Click the upload tab
    -Browse for the addon file (simple-cart-collect-customer-input.zip)
    -Click Install Now and then activate the plugin

    But the addon isnt available as above
    Anyone know Where I can download the “Simple Cart Customer Input” addon
