When I upload an image it stuck and create many entry on the database

When i upload a file in the media library it can happen that at the end of the upload, just when the green bar reach the end, but before displaying anything else, it goes in a loop and it starts to create hundreds on entry in the database under the posts table and the only way to stop it is to restart apache.

this is not the first time that this happened and also in different installations of wordpress. any idea on what the problem might be?
