Weird Activation Issue

I have a plugin that I wrote that works fine on my site, when activating/deactivating.. however, I sent it to someone, and when they activate it, it shows the php code and generates an error rather than actually executing the code.. any suggestions on how to fix it?

could it be as simple as changing the <? to <?php

?
