    my best bet; you’re using another plugin that also does JS and HTML optimization? can you (double-)check your W3TC settings to make sure all minification is off there?

    if that does not work, have a look at:
    * the HTML source of the page
    * your webserver error log
    * your php error log

    to see what that teaches us.
