Webinar form not submitting

Hello!
We have created a custom page for users to register for an upcoming webinar. The form is displayed directly into the theme template file using the shortcode provided in the FAQ section. The form shows up on the website, but when we go to test it, the page refreshes and nothing else happens. We double-checked the webinar register account and nothing is there either. I have read over a few posts and have emptied out our log, uninstalled/reinstalled the plugin, but nothing is working. I currently have the submission going to a different page for testing purposes, but that is not working either.

Another thing that is weird is if I go to a regular page and try to select the webinar form from the dropdown provided by the plugin, the list is empty.

Is there any other way to test why this is not working?

Any help that we can get from here would be greatly appreciated
