Web-to-Case/Web-to-Lead custom fields not populating

We purchased the Pro version of the plugin. When we try to map fields using Web-to-Case or Web-to-Lead, the additional form data (Account Name, Contact Name, Contact Phone, Contact Email) is not appearing in Salesforce when the Lead or Case is created.
Do you have a guide for setting up the Web-to-Lead or Web-to-Case in Salesforce? Any settings that need to be turned on in order for the plugin to work?
