WC Fields Factory Information Not Present at Checkout

We capture dog collar nameplate information via WC Fields Factory. We have been using this since 2018. Recently, the customer information is no longer appearing on the checkout page. It is present in the cart, but when the customer proceeds to checkout the information is lost. The information does not appear on the final order either so we are having to email customers who purchase the custom nameplate collars for their nameplate information. All settings are configured to appear in the checkout and as editable. Please provide a fix as soon as possible.
