Way to display form results automatically and immediately for an open discussion

We have a password protected page for individuals that after viewing an online video are asked a series of questions.

We would like those questions answered on that page and viewable to others that visit that page. We’d also like everyone to have the ability to comment…creating a discussion about answers.

I’m not sure if this is possible or if there’s a plugin I should have to help.

I’ve allowed comments on that page but I would rather create a form or something like that so that they are answering specific questions, not just giving comments.
