Was great but no longer working

This plugin was great at first! However, we encountered problems a while back and got no response to our support thread. Tried making a new form, re-saving Permalinks, disabling plugins, activating Twenty Sixteen theme, reinstalling the plugin… nothing works.

If it ever starts working again, would be happy to try it again. But until then, will have to find a different plugin
