“Warning” with Widget

I have created several forms using Nex-Forms, and most seem to work okay. However, 2 of the forms I created are not showing up on my website. I have added these 2 forms as widgets in the footer area of my website, but when I view the site, I get this warning message for both forms:

I don’t understand why this is happening. Can someone please help? The website is https://www.mylocalcomputertech.com. Thank you!
