“Warning – Failed to renew token” in logs

We’ve had an issue where users have to do a password reset a lot, and then the next time they go to log in they have to do it again. It’s happened to me as an admin as well.
I see “Warning – Failed to renew token –?retrying” in the logs, but I’m not sure what it means. Is there a disconnect happening that could be causing that issue?
In settings:
Checked: Force Login Redirects & Logout Everywhere
Unchecked: Force Disable Login Redirects & Logout Everywhere
Unchecked: Use Silent Redirection
(We use WP Fusion connected to ActiveCampaign, but passwords are not synced.)
