    I get the the above warning and blank page when trying to insert the quiz shortcode into a new page.

    The rest of the error is as follows (with URL taken out)

    No the quiz looks fine in the preview tab.

    It is more than likely a conflict with the theme or a plugin then. Which theme are you using? Can you give me a list of the plugins that you have installed?
