Want to add First Name and Last Name field in quiz form

1. Head over to the “QSM Quizzes and Surveys” section.
2. Navigate to the “Contact Form” tab.
3. Label [add first and last name prompt there].
4. Enable it and remember to save changes.

By implementing this, your quiz will prompt users to provide their first and last names effortlessly.

Moreover, you have the flexibility to decide whether to display the form at the beginning or end of the quiz. Here’s how:

1. Go to the “Options” tab.
2. Select the “Contact Form” subtab.
3. Under “Contact Form Position,” choose your preferred location.
4. Don’t forget to save your changes.
