W3T Cache settings

Can anyone tell me why w3tc is not storing cache files in the wp-content/w3tc/pgcache folder? I do have Page Cache on, and I have Cache Preload set on. Could those files be in the wp-content/cache/page_enhanced folder instead?
Also, I could use some advice on the Cache control settings. I have them set for “Cache-Control: max-age=3600, public, must-revalidate, proxy-revalidate” – is that the best setting? I’ve read up on it, but seen conflicting advice.
One last thing – I have a dedicated server, so should the Page Cache be set on the ‘Opcode’ setting rather than ‘Disk enhanced’?
