W3 Total Cache causes site to be messed up – minify options are off

I am trying to use w3 total cache for CDN purposes only, and do not need any of the minify options, and I have disabled all of them, or so I think, yet when I view my site the header is repeated several times and the columns with widgets are not displaying properly, either. When I have tried playing with minify settings in other plugins I have gotten these errors, so that’s what it looks like ot me, but I don’t think I have any minify options enabled.
On the General options page I DO NOT HAVE Page cache, Minify, Database Cache, Object Cache or Browser Cache enabled. I DO HAVE CDN enabled, and I have chosen to export the media library, theme files and custom files to AWS, and have configured those settings, and checked them with “Test S3 Upload” with success. I have uploaded the media library, theme files and custom files to AWS.
Automatically upload minify files is checked by default, and I can’t deselect it, however since I have chosen not to minify anything this should not have a negative performance impact on my site.

When I disable the plugin the site displays properly. I would like to use this plugin but can’t do so with these errors, and any help in correcting them would be greatly appreciated.
