W3 Total cache 2 quick questions here.

first how do i uncahce my forms pages? im using gravity forms and i read they have to not be cached to work right, im getting white screens. i see the box i use, but in what format do i write them in the box? the url or what?

just below the pic there is a description and reviews, they are supposed to be tabs, when you click and it changes you to one or the other, but now there not, how do i fix this? if i remove w3 it goes back to normal so it must be w3.
