Visual Image Editor not working – WP 3.5.2. Any suggestions welcome

Since installing WP 3.5.2 I’m unable to see/edit images in the visual editor (they appear as a small box in top left corner of visual editor screen without image)

When I try to find the files by following the image url – they are not loading and looking up on server via FTP shows the file to be missing from the (in this case “2003” folder)

I’ve reloaded all the WP files via FTP and gone for several clean slate reinstalls – same problem.

All plugins disabled + cache cleared through each reinstall.

Am running all files in the web root directory on server (not WP folder)

Tried in different browsers – same problem

Tried using default WP themes – same problem

Any suggestions on my next steps or possible errors in install welcome
