Is it possible to create a simple yes/no question survey for insertion on a website? I have five (5) simple questions that all require a “yes” or “no” answer.

If all five questions are answered “yes” I would like a pop up that says “CONGRATULATIONS” etc.

At that point I want a “continue” button that will lead to another, “yes” “no” question. I would then like another message to pop up, telling the person what to do next.

Nothing else is needed. No email. No tabulations. No contact info. Nothing.

Is there such a form/plug-in?

If not, how does a novice get this accomplished.
