Looks like that’s a major bug that I missed.

I fixed it in Version 1.2.1 so I recommend you update immediately.

Hmm, try deactivating the plugin, grabbing a fresh copy of 1.2.1, and reactivating. If that doesn’t work, I will explore more.

now, I can create form, but it’s all. I have blank fields with red border after I try save form and dont see any own names of rest fields

Those red borders mean that they are required fields. You cannot save the form without entering sender details.
