Very slow loading

Hi

My form – which can be seen at https://artists.flip.org.uk/application-form/ – is taking a very long time to load. The original Google Form is working fine and I’ve removed as many options as I can see but still no luck. I”ve turn caching on – which helps for reloads but I fear users won’t wait long enough for the form to load first time round.
