Very Critical Admin Log Out problem

I was trying to embed banner in the upper right side of my blog but I couldn’t succeed. I tried to test placing it on the themes functions and an error message occurred. I immediately deleted the banner code and re-update but the problem remain the same.

So, now my blog is open without logging out from admin.

Pls I need urgent assistance.
