Version 4.0 Broke Block IP Function, Causing 404/403s for ALL IPs

After upgrading to version 4.0, my sites using the plugin started returning 403s for all traffic. I determined that the Blocked IP redirections were causing this. ALL IPs are being caught by the first existing Block IP redirection rule. Disabling the Block IP redirections fixes it.

In addition, the Block IP function chosen after grouping by IP does not default to Regex anymore, which it should. Note the existing Block IP rules that no longer work properly do still have Regex selected.

Please fix both issues ASAP and release an update.
