vendor membership page not showing header

[wcfm_vendor_membership] is shown in this page https://www.gomyhealth.com/healthshop/vendor-membership/ and i cant find WCFM Admin settings – Page settings in the admin pannel there is marketplace and wcfm options

when i click on wcfm options that windiow is showing WCFM Page Settings withdropdown selected to store manager

my problem is that when you go through this page https://www.gomyhealth.com/healthshop/vendor-membership/ it’s not showing anything i meant theme styles and logo and menus all are missing i want to show menu and logo in this page
